People living with chronic illness, autoimmune conditions, long COVID, endometriosis, IBS or migraines; anyone waiting for a diagnosis who needs to show a pattern; and carers tracking symptoms for someone who can't easily describe them.
Getting the most from a symptom tracker.
Track the symptoms that affect your day
Choose the symptoms you most want to understand or discuss with a doctor: pain, fatigue, nausea, brain fog, dizziness. Eight is the maximum; four or five is easier to keep up.
Use a simple scale
Tick for present, or rate 0–3 (none, mild, moderate, severe). A consistent scale makes the month comparable at a glance.
Use the flare log for bad days
When a symptom is worse than usual, add a row to the flare log with severity, a possible trigger and what helped. Over time, triggers start to repeat.
Shade periods, travel or medication changes
Lightly shade the days of a period, a trip or a change in medication. It helps you and your doctor see what lines up with what.
Take three months to appointments
Specialists often want to see patterns over several months. Bring the pages, or export them from your note app, rather than relying on memory.

About this symptom tracker template
Need help importing it? See devices and apps or the FAQ.
Which symptoms can I track?
Any you like. The rows are blank so you can write your own, such as joint pain, fatigue, migraine, nausea, anxiety or skin flare-ups.
What's the flare log for?
Recording bad episodes in more detail: when, which symptom, how severe from 1 to 10, what might have triggered it and what helped. It's the part doctors often find most useful.
Is it suitable for chronic illness?
Yes. It's part of our chronic illness planner, alongside a medical profile, test results and a mood and energy tracker.
Can it diagnose anything?
No. It's a record to help you notice patterns and explain them to a healthcare professional. Seek medical advice for new or worsening symptoms.
