Guide

How to use a chronic illness planner

Living with a chronic illness means tracking a lot: symptoms, energy, medication, appointments and results. A chronic illness planner keeps all of it in one health binder, with pages that are quick to fill in on low-energy days.
  1. Fill in your medical profile

Conditions, allergies, medication, doctors and pharmacy on one page. Take it to every new appointment.

  2. Name your symptoms

On each month's symptom tracker, write up to eight symptoms down the side and rate them daily. A tick or a colour is enough on bad days.

  3. Log flares

The flare log has room for the date, symptom, severity, possible trigger and what helped.

  4. Track mood and energy

The mood and energy tracker shows how sleep, movement and time outside line up with good and bad days.

  5. Prepare for appointments

Write questions on the appointments page beforehand, and note results on the test results pages afterwards.

What to include in a chronic illness planner

A health planner should make tracking easier, not harder. This one includes:

Calendar

Weekly and monthly pages

Dated month calendars for appointments and a weekly page for each week's plan. No daily pages; the monthly trackers have a column for every day.

Records

Health Records

A medical profile, three test results pages, and a symptom tracker with flare log and a mood and energy tracker for every month.

  • Medical Profile · 1 page
  • Test Results · 3 pages
  • Symptom Tracker · every month
  • Mood & Energy Tracker · every month
Health

Health Admin

Appointments with questions to ask and a monthly medication and supplement tracker.

  • Appointments · 2 pages
  • Medication & Supplements · every month
Well

Wellness

Year in pixels, a self-care menu and a monthly wellness tracker.

  • Year in Pixels · 1 page
  • Self-Care Menu · 1 page
  • Wellness Tracker · every month
Sleep

Sleep

A monthly sleep log with bedtime, wake time and quality.

  • Sleep Log · every month

Every section can be switched off in the builder, and sections from other planners can be added.

Digital vs printable chronic illness planner

A digital health binder goes with you to every appointment on a tablet, and it's easy to show a doctor a month of symptoms at a glance.

Printed, keep the medical profile and the current month's trackers in a folder by the bed for days when screens are too much.

An example week in a chronic illness planner

One goal, followed through a single week. The milestone and weekly actions are the kind of lines the AI goal step writes onto your month and week pages; the daily notes are what you add by hand.

Goal

Track symptoms daily before my specialist appointment

Milestone for March

A full month of symptom ratings

This week's actions

  • Rate symptoms each evening
  • Log any flare the same day
  • Write three questions for the specialist
  • MondayFatigue 3, joint pain 2, brain fog 2. Slept 7 hours.
  • TuesdayFlare: joint pain 7 after a long day standing. Logged trigger and that rest helped.
  • WednesdayLow energy. Ticks only, which is fine.
  • FridayBlood test results added to the test results page.
  • SundayQuestions for the specialist written: three, from the month's patterns.

Questions

Chronic illness planner questions

Can I show it to my doctor?

Yes, and many people do. The symptom tracker, flare log and medication pages are designed to be easy to read at an appointment. Print the pages you need or show them on your tablet.

Is it medical advice?

No. It's a place to record what's happening. Decisions about treatment belong with your doctor.

Is there a free chronic illness planner?

You can build this chronic illness planner and flip through every page in your browser for free, with a watermark. The tabs and links work in the preview, so you can check the layout before you pay. The clean PDF is $12 once, or included in Pro at $49 a year for unlimited planners. There is no free download of the full planner.

Does it work with GoodNotes, Notability, Samsung Notes and e-ink tablets?

Yes. Download the iPad size for GoodNotes, Notability, Noteshelf, Xodo, Collanote or ZoomNotes; the Android size for Samsung Notes, Penly, Flexcil or Xodo; or the E-ink size for reMarkable, Kindle Scribe, Boox and Supernote. The links work in any app that follows PDF links, usually in read mode rather than pen mode. Every size is included in the price.

Can I print the chronic illness planner?

Yes. Choose A5, A4, US Letter or Half Letter in the builder for a printable PDF with a wider inner margin for hole punching. Printable and digital sizes come with the same purchase, so you can use the chronic illness planner on a tablet and print the pages you want on paper.

Is the chronic illness planner dated or undated?

Dated by default, starting in January 2027. You can pick any start month and year (including this month), a length from 3 months to 5 years, and Monday or Sunday weeks. Or switch to undated and write the dates in yourself, so a skipped week isn't wasted.

Does the AI fill in the chronic illness planner for me?

Only the goal parts, and only if you want it to. Type up to six goals and the AI writes a milestone for each month and a short action for each week onto your month and week pages, plus a motto for each month. You can edit every line before you download. Everything else, from the calendar pages to the trackers, is yours to fill in by hand. You can try it 3 times a day without an account.

How many pages is the chronic illness planner?

With the default settings it comes to about 158 pages. That includes 12 month pages, 53 weekly pages, the Health Records, Health Admin, Wellness and Sleep pages and 20 notes pages. Switching off sections makes it shorter, and the builder shows the total as you change things.

Make it yours in about a minute.

Set your dates, device and goals, flip through the preview free, and pay only when it's right.
