Five-Year Plan Template (Printable PDF & GoodNotes)

A five-year plan on one page: eight life areas down the side (health, career, money, home, relationships, learning, travel and fun) and five year columns across. Fill in where you'd like each area to be in each year.

Five-Year Plan template preview, shown on iPad

Shown for iPad in the Classic Navy theme. Printable downloads use white paper (there's a dark mode preview below); the e-ink version is black and white.

What's on it

One page, ready to fill in.

  • A table with columns for life area, year 1, year 2, year 3, year 4 and year 5
  • Undated, so you can print or duplicate it as often as you need

Who it's for

People at a turning point, such as finishing a degree, changing careers, starting a family or planning retirement, and anyone whose yearly goals feel disconnected from where they want to end up. Couples can fill one in together to compare where each of them wants to be.

How to use it

Getting the most from a five-year plan.

  1. Fill in year 5 first

Start with the right-hand column: where do you want to be in five years in each area? Then work backwards. Year 1 becomes the first realistic step towards year 5.

  2. Keep each cell to a few words

"Own a home", "run a marathon", "senior role". Short entries keep the grid readable and make it easy to update later.

  3. Leave some cells empty

Not every area needs a goal every year. Blank cells show where you've decided to maintain rather than push, which is a plan too.

  4. Write the real years above the columns

Replace "Year 1" with the actual year so the plan stays anchored when you look back at it.

  5. Review it every year

Each January or birthday, cross out what you've done, move what slipped and rewrite year 5. A five-year plan you revise is far more useful than one you write once.

Questions

About this five-year plan template

Need help importing it? See devices and apps or the FAQ.

How do I make a 5 year plan?

Decide where you want to be in each part of your life in five years, then break each one into a step per year. The template's grid does this for eight life areas, so you can see the whole picture on one page.

Which life areas does it cover?

Health, career, money, home, relationships, learning, travel and fun. Rename a row if one doesn't fit, such as faith or family instead of fun.

Is five years too long to plan?

It's long enough to plan big things like a career change or buying a home, and short enough to picture. Plans will change; the value is in deciding your direction.

Is there a full 5-year planner?

Yes. The 5-year planner has this five-year goals page, a five-years-at-a-glance page and dated monthly pages for all sixty months, plus goal, habit and review pages.
