Who it's for
Adults of any age who want to spare their family difficult decisions, people with a new diagnosis, older adults organising their affairs, and adult children helping a parent plan.
Getting the most from a final wishes.
Start with the easy boxes
Music and readings, or who to tell first, are gentler places to begin than burial or cremation. There's no need to finish in one sitting.
Write what matters, not everything
A few clear wishes help your family more than a long list. Say what's important to you and leave room for them.
Don't forget pets
Who will care for them, their routines and vet. It's often overlooked and causes real worry.
List digital accounts without passwords
Note which email, social media and online accounts exist and what you'd like done with them. Store login details separately and securely.
Tell someone where it is
Wishes no one can find can't be followed. Tell your executor or next of kin where the page is kept.

Is this a legal document?
No. It records your wishes to guide your family, but it isn't a will. Speak to a legal professional about a will, power of attorney and any legal documents.
Who should have a copy?
Your executor, next of kin or a trusted person. Keep the original somewhere safe that they know about.
When should I fill it in?
Any time. Many people do it after a big life change, such as marriage, children or retirement, and review it every few years.
What's in the end of life planner?
Final wishes, accounts and assets, key people, a medical profile, an emergency plan and a letter to a loved one, plus contacts and health pages.
