Who it's for
People with type 1 or type 2 diabetes who test by finger prick, those with gestational diabetes asked to keep a log for their midwife, and carers recording readings for a child or older relative.
Getting the most from a blood sugar log.
Write your targets at the top
Note the target ranges your care team has given you, and whether you measure in mg/dL or mmol/L, so every reading on the page has context.
Log every reading straight away
Write each number as soon as you test. Readings from memory at the end of the day tend to be rounded or missed.
Use notes for food, exercise and illness
A big meal, a walk, a missed dose, a cold or stress can all explain an unusual reading. A few words in the notes column helps you and your care team spot patterns.
Circle readings outside your range
A quick circle around highs and lows makes them easy to find when you or your doctor scan the page.
Bring the month to appointments
Two pages cover a whole month, which is usually what a diabetes nurse or doctor wants to see between visits.

What times does it log?
Fasting (first thing), before lunch, before dinner and bedtime, which are the four readings most care plans ask for. If you test after meals, use the notes column or the before-meal column with a note.
Does it work for gestational diabetes?
Yes, as a personal record. Your midwife or diabetes team may want after-meal readings too; write those in the notes column or ask which times they want recorded.
Can I record insulin doses?
Yes. There's a meds and insulin column for each day.
Is it medical advice?
No. Follow your care team's targets and advice. If you have a very high or low reading with symptoms, follow your hypo or hyper plan or seek urgent help.
