Who it's for
People with high blood pressure monitoring at home, those starting or adjusting blood pressure medicine, pregnant women asked to check readings, and anyone asked by their GP to record a week or two of home readings before an appointment.
Getting the most from a blood pressure log.
Measure at the same times each day
Morning and evening readings at roughly the same times give a fair picture. Write the time in its own column, because blood pressure varies through the day.
Sit quietly first
Rest for a few minutes before measuring, with your back supported, feet flat and your arm resting at heart level. Avoid caffeine, smoking and exercise just before.
Record which arm
Readings can differ between arms. Use the same arm each time unless your doctor says otherwise, and note it in the arm column.
Note anything unusual
Feeling unwell, a missed dose, a stressful day or a new medication all go in the notes column, so an odd reading has context.
Bring the log to every appointment
Home readings help your doctor see how your blood pressure behaves outside the surgery. Follow the targets and advice they give you.

About this blood pressure log template
Need help importing it? See devices and apps or the FAQ.
What columns does the blood pressure log have?
Date, time, systolic (top number), diastolic (bottom number), pulse, arm (left or right) and notes.
How many readings fit on a page?
Around 30 rows on the printable versions, enough for two readings a day for about two weeks, or one a day for a month.
Should I write down every reading?
Write down every reading you take, including high or low ones, unless your doctor has asked you to take several and record the average. Don't leave out readings that look wrong.
Is this medical advice?
No. It's a record for you and your doctor. If a reading is very high or you have symptoms like chest pain or severe headache, seek urgent medical help.
Can I use it for more than one person?
Use a separate page per person, with their name at the top, so readings never get mixed up. Couples and carers often keep both pages in the same folder.
