A CPD and continuing education log with four columns: date, course or training, hours and certificate. Record every course, conference, webinar and training session so you're ready for revalidation, licence renewal or an appraisal.

CPD Log template preview, shown on iPad

Shown for iPad in the Classic Navy theme. Printable downloads use white paper (there's a dark mode preview below); the e-ink version is black and white.

What's on it

One page, ready to fill in.

  • A table with columns for date, course / training, hours and certificate
  • Undated, so you can print or duplicate it as often as you need

Who it's for

Nurses and midwives preparing for revalidation, US nurses tracking CE hours for licence renewal, teachers, pharmacists, therapists and other professionals with annual training requirements.

How to use it

Getting the most from a cpd log.

  1. Log it the same day

Write the training down as soon as it's finished, with the exact hours. Working it out from emails a year later is painful.

  2. Be specific about the activity

"Sepsis update – online module – Trust" tells an assessor more than "training".

  3. Tick certificates and file them

Tick the certificate column when you've saved the certificate, and keep them in a folder in the same order.

  4. Add a running total

Every few rows, total your hours. You'll always know how close you are to your requirement.

  5. Include informal learning

Many bodies accept reading, reflective practice and peer discussion. Check your regulator's rules and log those too.

  6. Plan next year's learning

At your appraisal or renewal, look at the log for gaps and write two or three courses you'd like to do next year at the bottom of a fresh page.
