Getting the most from a iep goal data sheet.
Use one sheet per goal
Keeping each goal on its own sheet makes progress easy to read and easy to graph for progress reports.
Copy the goal exactly
Write the annual goal word for word from the IEP, including the criteria, such as 80 percent accuracy across three consecutive sessions.
Record trials and correct
Write how many opportunities the student had and how many were correct, then the percentage. Percent alone hides small samples.
Use consistent prompt levels
Agree a code with your team, such as I (independent), V (verbal), G (gestural), M (model), P (physical), and use it every time.
Keep initials only
Use initials or a code, never full names, and store sheets securely in line with your school's privacy rules.
Graph the percentages
Every couple of weeks, plot the percentage column on a simple line graph. A visual trend is easier to share at meetings and shows quickly whether a goal needs a new strategy.

What is an IEP goal data sheet?
A sheet for collecting regular data on a single IEP goal, so you can show progress, decide on next steps and write progress reports.
What do the columns record?
Date, number of trials, number correct, percentage, prompt level and notes.
Can paraprofessionals use it?
Yes. The simple columns make it easy for aides and therapists to record data consistently.
What's in the special education teacher planner?
IEP at a glance, IEP goal data sheets, ABC behaviour logs and daily lesson plans, plus teaching pages and contacts.
How often should I collect data?
